Please complete your application by filling out your answers to all of the required questions. The application consists of 4 sections in total and you can navigate to the next page by clicking the “next” button in the bottom right or by using the tabs at the top of the page.

If you need to save your application to return to it at a later time—or just to ensure you do not inadvertently lose what you have entered—please hit the “save” button at the bottom of the page. You will be given a number of options to sign up to Jotform via email, google, Facebook, or you may also skip the sign-up process by clicking “Skip Create an Account,” at the bottom of the window. Whichever option you select, you will be prompted to enter an email address and will then receive an email from [email protected] containing a button that can be used to return to your application.

Please note that you do not have to save every time you move to a new page on the application form. You only have to save if you are planning to close the form to return to it later. Nonetheless, in order to prevent any mishaps, we do recommend hitting the save button before you move on, and/or saving your work in a separate document (e.g., our downloadable application form).

Once you have completed the application please ensure that you hit the “submit” button on the bottom right corner of the final page. You will receive a confirmation email with a copy of your application. Once you have submitted the application, you cannot edit it any further. If corrections are needed, please email them to the secretariat at [email protected].

If you do not receive the confirmation email, check your junk mail. If you run into any technical difficulties with the application, please reach out to the secretariat at [email protected].

Your application must be submitted no later than May 16, 2022.
